It’s your responsibility to keep your employer informed of your work status following injury or surgery. It is also important that we keep records of your work status in order to be able to answer any questions your insurance company may ask. Please request a work status report at each visit, and make sure that you give a copy to your employer.

Disability

Hand to Shoulder Specialists of Wisconsin can complete your disability forms for you, but this is an additional service that does not impact your medical care. Therefore, we charge a fee for each form, which must be paid in advance. Please allow 7-10 days for completion.

Worker's Compensation

According to Wisconsin law, if you have a work-related injury or disability, you can receive payment for reasonable medical expenses and compensation for lost wages. In order to qualify, begin by filing a claim with your employer.
